WSAVA Prize Draw Terms and Conditions

1. Applications open to UK residents aged 18 Employees of the Allianz UK group, and their respective subsidiaries and associated companies, their immediate families and anyone involved in the promotion of the competition are excluded from entry. 2. Only one application per person. Allianz will disallow duplicate entries where it finds them. No purchase is necessary to enter. All entrants consent to our contacting them about our products. 3. Applications are only valid if entry form is fully completed. If a winner provides incorrect information that precludes contact by Allianz, the winner forfeits the prize. The winners’ name shall be made available on request. 4. Closing date 16.04.10, the prize will be awarded to the first entry selected at random and the winner will be notified by 30.04.10. The prize shall be awarded at Allianz’s discretion and if it considers any entrant has acted inappropriately or dishonestly in entering the competition it reserves the right to refuse to award any prize. 5. The winner will be notified by phone by 03.05.10. In the event of the prize being unavailable for any reason outside Allianz’s control it reserves the right to offer an alternative prize of equal value. 6. The prize consisting of: 2 tickets to WSAVA Congress 2010 to be held in Geneva from 02.06.10 to 05.06.10 including economy flights from the UK to Geneva, and 3 nights’ accommodation including breakfast and £100 spending cash. Excluded from the prize are transfers to the specified airport or airport parking charges in the UK and/or Geneva. Allianz is not responsible for the quality of the services or goods supplied. 7. The prize tickets/vouchers and spending money will be sent to the winner by courier/post no later than 17.05.10. The prize may be taxable and the winner will be responsible for any tax arising. 8.The winner must be in possession of a valid passport to meet relevant entry requirements and obtain any required visas for European travel at the time of travel at own expense 9. The prize is non-transferable and there is no cash or other alternative. 10. The promoter’s decision is final and no correspondence will be entered into. 11. All entrants are deemed to have accepted and agree to be bound by these Terms and Conditions. Allianz reserves the right to change, modify, add, or remove portions of these terms of use at any time. The promoter of this prize draw is Allianz Insurance plc (“Allianz”) whose registered office is at 57 Ladymead Guildford Surrey GU1 1DB which is authorised and regulated by the Financial Services Authority. Petplan is a trading name of Allianz whose trading address is Great West House (GW2), Great West Road, Brentford, Middlesex TW8 9DX. 12. You agree that you may be filmed/ photographed during the WSAVA Congress and you hereby grant the promoter and its respective affiliates and subsidiaries the right to use any resulting film material or recording containing image, statements, voice and the participation picture for internal and external communication. 13. These terms and conditions shall be governed by the laws of England and Wales and any action arising out of these terms shall be litigated in, and only in, courts of England and Wales, and you hereby agree to submit to the exclusive jurisdiction of those courts and further agree that they are a convenient forum for you.
